The Vantor login account and active access tokens.
Eligible profile and contact information linked to the account.
App preferences and eligible user-created content controlled by Vantor.

What may be retained

The deletion request and verification record needed to prove that the request was handled.
Security and audit records required to protect users and the platform.
Clinic-controlled appointment, treatment, communication, media, or clinical records that the clinic or applicable law requires to be retained.

This form requests deletion of the account and its associated eligible data. It does not request deletion of selected data while keeping the account active. Any retained information is restricted to the purpose that requires retention and kept only for the applicable legal or clinic-controlled retention period. We explain the outcome and any required retention when the request is reviewed.
